Record what\nhappened. Track its status. Report it upward. Most platforms live\non those three rungs, and\n[records are not decisions](/insights/records-are-not-decisions)\ncovered what stopping there costs.\n\nThe ladder has eight rungs. The upper five are not more of the\nsame climb. They are a different sport, and the industry's\nmaturity models quietly help hide that.\n\n## The upper five rungs\n\nEverything on the lower three rungs can be built with a database\nand a workflow engine. Each rung above carries an obligation the\none below never had.\n\n**Predict** obliges the system to say what happens next, with\nodds, on the record. **Detect** obliges it to notice a material\ndeviation early, out of millions of events, without a human\nwatching. **Explain** obliges attribution to drivers someone can\nact on. **Intervene** obliges a change of course while changing it\nstill matters, through a named owner with a deadline. **Prevent**\nobliges the loop to close, with proof the recurrence rate fell.\n\nThe upper five make one demand the lower three never do. They\nrequire the system to state claims that can be wrong, and so\nrequire the machinery of being wrong in public (e.g. calibration\nand backtests). A report can be late. A prediction can be false.\nThat asymmetry is the whole difficulty, and the whole value.\n\n## Where the maturity models stop\n\nVendor maturity models top out one rung early, at some formulation\nof AI-enabled insight. Insight is a comfortable summit because no\ninsight has ever been audited against outcomes. But a prediction\nnobody acts on is a report with odds attached. Your incident rate,\nforecast error and compliance findings can sit exactly where they\nwere while insight accumulates, and the gap between knowing and\nacting\n[compounds as decision debt](/insights/everyone-has-ai-few-get-value).\n\n*Insight is the entry fee. Prevention is the product.*\n\n## The one-question audit\n\nAsk one question of any system you own, this quarter, without a\nconsultant. What changed in the world because the software knew?\nNot what was reported or flagged, but what decision was taken\nearlier, what loss did not happen, what number moved. Try it on\nthe incident system, the audit platform, the planning suite. In\nmost rooms the honest answer is silence. Systems on the lower\nrungs have no answer.
